Lower GI
RANDOM:
- Most water is absorbed in the small intestines (27 feet long)
- Lower GI starts at the duodenum
- Terminal ilium absorbs B12 and intrinsic factor
- Normal flora is heathy, antibiotics can knock out your normal flora (e-coli)
- Large intestines makes: vitamin K and stores: bacteria houses: lubrication
- Normal bowel sounds are high pitched and gugguls
ASSESSMENT:
- Smoking, alcohol, water source (spring is better than well)
- Travel history (travel diarrhea)
- Surgical history: scar tissue due to adhesions
DIAGNOSTIC TESTING:
- Stool collection w/o toilet paper OR urine since it harbors other bacteria
- No red meet 3 days prior to test
- Iron preporation
- Vitamin C can cause false negatives
STOOL C&S:
- Avoid urine!
- 2 hours
- SBTF will be NPO
BARRIUM ENEMA:
- Day before they use laxatives
- Clear liquid diet the day before
- It is ok for it to be yellow
- If pt might have diverticulitis, they do not get a barium enema
- No more than 1,000ml and 3 in 24 hours since water is hypotonic and it draws
(3,000 total)
o Tepid water ;-)
- KUB (Kidney, ureter, and bladder) they might be broad-spectrum anabiotic’s
- MRI: NO METAL!
LOER GI ENDOSCOPY:
- Anoscope: anus to sequim
- Proctoscopy
- Sigmoidoscopy: first at age 45
- Colonoscopy:
LGI ENDOCOPY:
- Need a consent for this
- NPO the night before
- False positive can occur
