1A - ANS-LOC
0 = alert
1 = minor stimulation
2 = repeated stimulation
3 = coma or reflex only
\1B - ANS-LOC Q's: age and month
0 = both correct
1 = one wrong
2 = two wrong
* physical or language barriers are scored 1
* comatose or aphasic scored 2
\1C - ANS-LOC commands: eyes, fist (or other commands)
0 = both correct
1 = one wrong
2 = two wrong
* count it if they make a visible effort
\2 - ANS-GAZE:
0 = no paresis
1 = partial gaze palsy, cranial nerve palsy, or deviation which can be overcome with
VOR
2 = forced deviation or paresis not overcome with VOR
\3 - ANS-VISUAL:
0 = normal
1 = partial hemianopia or quadrantanopia
2 = complete hemianopia
3 = bilateral hemianopia / complete blindness (of any cause)
* if one eye is damaged, only test good eye
* if extinction present, give score of 1
\4 - ANS-FACIAL:
0 = normal
1 = minor paralysis
