situation. Heart tones are regular with no murmur. Lungs are clear to auscultation. Abdomen is soft with active bowel sounds. Capillary refill is brisk in
extremities. No tingling or numbness is present. He states that he doesn't know why he has lost weight since he is not
dieting. Client states he becomes fatigued as the day progresses.
