Assessment Check List
Skills
Prepare environment, gather equipment, maintain medical
asepsis
Head:
• Inspect and palpate scalp
• Inspect head and face
Eyes:
• Inspect eye lids and anterior eye
structures (conjunctiva, sclera, pupil,
cornea, iris)
• Assess visual acuity (Snellen chart)
• Assess pupillary reaction to light
• Assess visual fields on confrontation
• Assess extra-ocular eye movements
• Perform fundoscopic exam to inspect posterior
eye structures:
✓ Red reflex
✓ Optic disc, disc-to-cup ratio
✓ Retina: arteries, veins, fovea, macula
Ears:
• Inspect and palpate external ear (auricle, tragus)
• Inspect ear canal, TM using otoscope
• Test auditory acuity with whispered voice test
Special tests if abnormalities on whispered voice test:
✓ Weber test
✓ Rinne test
Nose:
• Inspect anterior and interior surface of the
nose (nasal turbinates, mucosa, septum)
Mouth and Throat:
• Inspect oral structures (teeth and gums,
tongue, roof and floor of the mouth, uvula,
tonsils, posterior pharynx)
Sinuses:
• Palpate frontal and maxillary sinuses
