NURS 6501N Pharmacotherapeutics- Right Upper Quadrant Pain
Pharmacotherapeutics: Right Upper Quadrant Pain Patient DC presents to the emergency room with the symptom of right upper quadrant pain after having a large meal known as biliary colic which is typically a severe crampy pain in the right upper quadrant (RUQ), after having a large meal. The pain gradually increases and is commonly associated with bringing up vomit as noted in patient DC, (Rosenthal & Burchum, 2021) Due to the general symptoms concerning gastrointestinal conditions, the first step for patient DC will be to confirm a diagnosis that will warrant the correct treatment that she needs for her symptoms. Since the most common cause of right upper gastric pain involves gastrointestinal lOMoARcPSD| issues, it would be appropriate to address the abdominal pain symptom and advise the patient not to take anything by mouth (NPO). While planning for testing, the patient can be started on intravenous fluid therapy to prevent hunger and thirst. Tests will be the second most important thing to get a confirmation. The differential tests should include CMP to include Helicobacter Pylori testing, (Tarhini et al., 2018). Endoscopy, and radiology of the GI system. The diagnostic techniques for H. pylori infection are categorized into invasive and non-invasive. The invasive techniques include endoscopy, histological studies, culture, and rapid urease test whereas 13Curea breath test, stool antigen test, blood test, serology, and PCR are included in non-invasive techniques, (Zubair, 2015). In order to keep the patient comfortable and symptoms under control, the first medication I would recommend is, 1. Proton pump inhibitor agents – Dexlansoprazole/Dexilant - 60mg capsules once a day, due to its ability to suppress acid, inhibit H+, and K+-ATPase, gastric acid-producing enzymes - Relive pain by reduction of gastric acids to prevent gastrointestinal bleeding - and educate the patient on the expected symptoms of drowsiness, weakness, and restlessness. The patient may have some confusion and hallucination 2. Due to the suspicion of infection and inflammation of the stomach, I would recommend, giving an antibiotic. - Amoxicillin 500 mg capsules 1 by mouth every 6 hours
