Clinically it is divided into two types:
1- Tubotympanic type (safe)
2- Attico-antral type. (unsafe)

, Tubotympanic type
⚫ Etiology: follows ASOM as a result of :
1) Organism factors:
a) Recurrent infection e.g. adenoiditis and
sinusitis.
b) High virulence e.g. exanthemata.
2) Treatment factors:
a) Inadequate dose or short course.
b) Inadequate drainage .

⚫ Multiple organisms both aerobic and
anaerobic.
⚫ Pseudomonas aeruginosa.
⚫ B.proteus.
⚫ E.coli.
⚫ Staphylococcus aureus.
⚫ Bacteroides fragilis and streptococcus.
