SKIN
Begin with an overall visual inspection of the Skin surface.
Color and texture is uniform, unblemished, no foul odor, with no signs of lesions, masses,
infection, or infestation.
Body hair is minimal and evenly distributed
Skin turgor is good; Temp within normal limits
Finger Nails: are light pink with the shape of convex curve; smooth and intact with the
epidermis; capillary refill <2 seconds.
Feet: are without skin breakdown, nail discoloration, ulceration, or infection
HEAD
Midline; Round; Normocephalic; and Symmetrical
Palpate Skull: without nodules, masses, or depressions when palpated
EYES
Snellen chart one eye at a time CN 2
Peripheral Fields – Confrontation Test – 3 feet away – Cover opposite eyes – wiggle fingers
into the center from the side
Six Cardinal Fields of Gaze – no sustained or jerking nystagmus CN 3,4,6
Eyelids: no discharge, discoloration; lids close symmetrically
Conjunctiva: shiny, smooth, and pink
Cornea: smooth and transparent; blinks when touched CN 5
Pupils: black and equal in size
PERRLA: Illuminated and Non-illuminated pupils constrict; Pupils constrict at near object and
dilate at far object; Pupil converge when object moved towards nose
Ophthalmoscope – Red Reflex – No Opacities
Retina: orange/red in color
