EHR Implementation
In order to overcome the challenges presented by an outdated and disorganized hybrid
(paper/digital) EHR system, Anywhere Memorial Hospital will be implementing a fully digital
system. The CEO, the medical director, the HIM director, and the HIM coding supervisor are
among the stakeholders who will be charged with identifying the problems with the current
system, analyzing the requirements that will be necessary for the change, building process, and
workflow strategies, oversee the installation of the system, enforce adherence to policies
governing the use of the system, promote training for all users, and maintain the system,
upgrading as necessary.
ISSUES IN THE HYBRID RECORD
One challenge of the hybrid record system is the timely completion of clinical charting.
The legacy physicians will be in a perpetual “limbo” with some documentation on paper and
some charting done on the EHR system. The HIM personnel must take extra time to gather, scan,
and enter the paper notes into the records, often hours or days after the encounter with the
, patient. The records must then be checked for accuracy and verification. (Puttgen, et. al, 2021)
The foundation of the revenue cycle is clinical documentation. The necessity of services
provided must be verified and matched to the patient record prior to billing. Hospital charges and
professional fees depend on the timeliness and accuracy of the documentation that begins with a
patient’s registration. When delinquent charting data delays the timely completion of records
necessary to payers within the time limits set, revenues are lost when payment is denied. (Oachs,
2020)
