specifically biological mother and father
only diabetes and hypertension
--if no, click ... and then denies whatever it is

social history
specifically ask about tobacco
, --click details in substances, then click value drop
click which ever where is applicable
----if they smoke, provide counseling yes and date ... dont have to ask details
if they are under 16, ask household tobacco
if BMI or BP are in red, then you add more info here
