EVALUATION
After carefully reviewing the tracer report, it was noted that the H&P was not done within
24 hours of admission into the hospital. This report was done more than 72 hours later. In order
to provide the best quality and patient safety, it is imperative to inform the condition of the
patient when admitted to the hospital.
CORRECTIVE ACTION PLAN
Joint Commission Standard: PC.01.02.03 Hospital assesses and reassesses the patients’
condition based on their designated time frames.
