VMED 5267 - Pain
Management
1.• Especially (but not limited to) surgical pain
• Before, during and after painful stimulus
• Reduces doses of analgesics required compared to treating only 'after'
stimulus is created
Prevents central sensitization: When and why should you do preventative anal- gesia?
2.Inhibit impulse condition
- Local anesthetics
- a2-agonists: What analgesics inhibit transmission?
3.Inhibit peripheral sensitization of nociceptors
- NSAIDs
- Opioids
- Local anesthetics
- Corticosteroids: What analgesics inhibit transduction?
4.Inhibit central sensitization
- Local anesthetics
- Opioids or a2-agonists
- NSAIDs
- NMDA antagonists (ketamine)
- Tricyclic antidepressants
- Anticonvulsants: What analgesics alter modulation?
5.- Anesthetics
- Opioids
- a2-agonists
- Benzodiazepines
- Phenothiazines: What analgesics inhibit perception?
6.Post-surgical pain
• Coadministration with an opioid in the immediate post op
• Not for intraoperative pain management !!!!
Non-surgical pain caused by inflammation
• Meningitis
• Tumors (bone)
• Mastitis
• Cystitis
• Dental pain: What are indications for using NSAIDs?
7.• Shock or trauma upon presentation
- Rule out hemorrhage before
