1. Based on the most urgent problem and priority actions/interventions identified for
this patient, what additional assessment data would you need to effectively care for
this patient?
a. The urgent problem when caring for this patient was the new pressure injury
that was present on his left heel. The priority action I identified was to alleviate
the pressure by using a cushion to elevate the area and to avoid applying
pressure to the area for long periods of time. Additional data that I would need
to effectively care for this patient is to assess his nutritional status and moisture
exposure. Additionally, evaluating the patient’s Braden Scale score and lower
extremity circulation will help determine their overall risk and come up with
appropriate interventions.
2. Choose one environmental and/or individual factor that could impact the patient’s
ability for post-operative recovery once they return home and discuss ways to mitigate
(reduce the impact of) it.
a. One environmental factor that could impact the patient’s ability for post-
operative recovery is their support system. Having a support system can play a
big role in the patient’s recovery process since an individual can provide
emotional support, assistance with ADLs, and making sure the patient is
following post operative plan of care. This patient from the simulation lived at
home alone since his wife has been living at a nursing home for the past two
years due to end stage Alzheimer’s disease.

a. What would be the plan of action if a patient refused to attend physical
therapy after surgery even if they were informed of the risks?
