Identify at least TWO assessments that can be used to determine whether a client is safe in
their current living environment.
Cao et al. (2021) state that an increased number of older adults choose to stay home as they
age. However, home safety for older people is becoming more challenging globally. Elderly
people are at higher risk of any safety-related events than younger populations because of the
natural aging process and health status with various chronic medical conditions (Cao et al.,
2020). The two assessments that determine whether the patient is safe in the current living
environment are the Home Falls and Accidents Screening Tool (HOME FAST) and the Home
Safety Self-Assessment tool (HSSAT). Ahmad Ainuddin et al. (2022) state that HOME FAST is an
appropriate tool to use in fall prevention for the older population, aiming to identify the
environment and the risk factors related to falls. Davis & Davis (2022) state that HSSAT is a self-
assessment tool for patients and caregivers to self-identify and modify potential environmental
fall hazards (David & Davis, 2022).
Discuss results that indicate the client may require additional assistance or a transition in the
care environment.
Lee et al. (2022) state that the American Housing Survey suggests that if a patient lives alone at
home can put the patient at high risk of falling. Factors that can prevent older adults from living
at home safely include any age-related chronic diseases, frailty, and disability that can prevent
them from functioning independently. Therefore, it is recommended that the patient have a
home safety assessment to determine the patient's capabilities and the home environment to
identify any problems that could affect the individual’s quality of life (Lee et al., 2022).
Describe the NP’s role in working with the client and family to assist with care transitions.
The nurse practitioner plays a significant role in working with the patient and the family in
assisting with care transitions. Earl et al. (2020) state that healthcare provider involvement can
contribute to safe and efficient care transitions. It is the health care provider's responsibility to
establish a care plan that includes patient and family education, effective communication, and
transfer of information to other facilities or providers about all appropriate information
regarding the patient's health status, considerations, and medications and to facilitate the
continuity of care by establishing follow-up appointments (Earl et al., 2020).
Identify at least TWO appropriate referrals for a client who requires a transition in the care
environment. How does the NP ensure coordination of care during transitions?
The appropriate referrals for a patient who requires a transition in the care environment are a
social worker referral and a referral to a home health agency or skilled nursing facility. The social
worker can determine the patient's need, provide necessary resources, and help in the care
transition. The nurse practitioner’s role in the care transition to a facility is to ensure all
