Reason for encounter: Bumps on face (likely acne or another
dermatologic condition)
Subjective
Chief Complaint (CC):
"I have bumps on my face that won’t go away."
History of Present Illness (HPI):
A 14-year-old male presents with a 3–4 month history of progressively
worsening facial bumps. He notes that the lesions began on his
forehead and have spread to his cheeks and chin. They are sometimes
red and mildly tender but not itchy or painful. No drainage, bleeding, or
scabbing is reported. The patient denies fever, fatigue, or recent illness.
He has tried over-the-counter facial cleansers with minimal
improvement. He denies using any topical creams, medications, or
home remedies. No known allergies. He reports no picking or squeezing
of the lesions.
