j. Pharmacist comments relevant to the individual's drug therapy, including any other
information peculiar to the specific patient or drug.



Pharmacists shall be required to make a reasonable effort to counsel the patient or the
patient's caregiver in person or by telephone when dispensing the first fill of a new
prescription for the following: - ANSWER (1) Prescriptions for patients under the age of
13;



(2) Concentrated medications;



(3) Anticoagulant/antiplatelet medications;



(4) Endocrine medications; and



(5) Anti-infective medications.

Notice of Closure - ANSWER (a) Written notice to the board shall be submitted at least
15 days in advance of the date of the intended closure. The notice shall specify the date

(b) Notice in writing to the DEA shall be submitted no less than 15 days before the date
of the contemplated closing. Obeying DEA directions with respect to closure
procedures will be mandatory.

(c) At least 5 days prior to the anticipated closing a notice shall be conspicuously posted
at the pharmacy indicating the date of closing and the future location of the prescription
files. This notice shall be posted for a period of at least 30 days unless removed by the
landlord or a new tenant.



DEA 222 form - ANSWER Ordering and transfering CII's



No later than 20 days after a pharmacy closing, the licensee shall: - ANSWER (a) Return
the pharmacy permit to the board;



(b) Notify the board that all signs and symbols indicating the presence of a pharmacy
have been removed;



(c) Notify the board that all labels and blank prescriptions have been destroyed;

(d) Provide written notification to the Board that the DEA license and all blank DEA #222
forms have been returned to the Regional Director of the DEA;



(e) File with the board, a copy of the dated inventory of all controlled substances
transferred including the name and address of the person(s) to whom these drugs and
applicable records were transferred; and
