Two methods:
1) Z-plasty: create two triangular flaps, incision along the scar, changes orientation of the scar (perpendicular to
previous). We can choose the angle of the flaps, the greater the angle, the lengthier the scar. It recruits adjacent
tissue to the scar reducing skin tension.
2) Autologous fat grafting: take fat (generally thigh or abdomen) from the patient and inject it into another
region of the body. Restores volume.

Definition of grafts and flaps
• Graft = healthy tissue (ex. skin, cartilage, bone) harvested from one body area (donor site)
and transplanted to another (recipient bed), without its own blood supply.

• Flap = maintains its own blood supply and is therefore transferred with a pedicle (portion of the tissue
that ensures its nourishment as it is composed of an artery, vein, and sometimes also nerves)

History of grafts
1500 BC India = skin graft use for traumatic nasal amputation

1804 (Baronio) = skin graft in a lamb
1840 (Warren and Pancoast) = described first autologous skin graft in humans
Ollier (1872) and Thiersch (1886) = human split-thickness skin graft (STSG)
Wolfe (1893) introduced the concept of human full-thickness skin graft (FTSG) which was further detailed
by Krause (1893).
