What happens when there is an injury in the workplace? - answer If there is an injury
that requires employee to go to hospital, submit report to HR generalist to begin a
workers comp. claim. If it is not an emergency, the employee must notify HR to initiate
the claim. If there is no medical assistance required, a report must be submitted to HR
regardless so that it can be documented
What goes into a Workers Comp. Checklist? - answer○ If necessary, complete LDR in
the event of an auto accident
○ Contact HR to obtain proper documents: Workers Comp 1st Claim and Workers
Comp Claim Form DWC-1. These must be completed by the employee and BM. Signed
copies will be provided to both parties.
○ Fax forms to Sedgwick
○ Employee must visit approved medical care center (Concentra/Doctors on Duty)
Employee can't return to work without a doctor's authorization
What is the difference between Full Time PTO and Part Time PTO - answer○ Full Time
Start with 5 days and gradually increase to 13 in Year One. Full time hours accrue a
fraction with each day worked. Part time accrues a fraction of every hour worked.
What is the rule with how lunch breaks must be distributed? - answer○ Lunch breaks
must be taken before the end of the 5th hour worked
What is Alternate Work Arrangement (AWA) and how does it work? - answer○ Working
with an employee to make their time and development successful, such as ensuring that
they are allowed to leave early on certain days to ensure they can attend a class or
