Short Answer
1. NEC—Not Elsewhere Classifiable indicates that a more specific code is not available even
though the diagnostic statement indicates a more specific condition.
NOS—Not Otherwise Specified indicates that the code being used is an unspecified code
based on the diagnostic statement.
2. The Alphabetic Index will identify the category a code selection can be made from.
Referencing the Tabular List will verify code selection and also identify any instructional
notations that might apply to proper code selection.
3. Excludes1—a pure Excludes note. It means “Not Coded Here.” An Excludes1 note is used
when two conditions cannot occur together, such as a congenital form versus an acquired
form of the same condition.
Excludes2 represents “Not Included Here.” An Excludes2 note indicates that the condition
excluded is not part of the condition represented by the code, but a patient may have both
conditions at the same time.
4. K75—Other inflammatory liver diseases
Excludes2: Toxic liver disease (K71.–)
Students could pull any number of different examples for this particular question.
5. The See instruction after a main term in the Alphabetic Index directs the coder to reference
another term for further clarification of a more precise code.
