Interprofessional Collaboration Reflection
Hello, everyone. I'm Ma Khrysia Anne De Guzman, a Community Mental Health
Center registered nurse. The purpose of today's self-reflection will focus on our
interprofessional collaboration in delivering mental health services to culturally diverse
patients. These comprise psychiatrists, psychologists, social workers, nurses, and community
health workers. We do our best to ensure the continuity and integration of care; however, we
can identify opportunities for enhancing collaboration with other providers in the patients'
management. In this video, I will focus on our present partnership, constraints encountered in
serving patients, and recommendations based on best practice guidelines that will help
improve our cooperation and, ultimately, patients' health.
Current State of Interprofessional Collaboration
Our interdisciplinary team has achieved notable successes in working together to
assess, diagnose, treat, and support patients with mental health conditions. Depending on the
case and the diagnosis we are addressing, we are filled with different knowledge and
experiences. Everyday care activities include medical management, which is given by
psychiatrists such as Dr. Nowak, including prescriptions of the required medication and
monitoring the changes that may be needed. Healthcare providers like Dr Connor prescribe
proper counselling services and use different approaches in psychotherapy when handling
patients. Lena Schmidt and other social workers ensure that the patients are linked to
necessary community resources and that every aspect that significantly affects mental health
from a social context is well addressed. This paper will describe the role of a nurse, which
includes patient care, educating patients, and acting as a liaison between the patients and
other team members. Administrative decision-makers need to appreciate the roles of CHWs
as cultural mediators, such as Jonathan Singh, who helps gain the trust of several patients.
