1. Describe the innovation. Describe the innovation and the problem it addresses.
The overall purpose of the Hackensack ACO project was to reduce readmission rates for
heart failure patients with more closely monitoring in the outpatient setting. The
consumer-focused venture was an innovation to furnish patients with internet-enabled
tablets to help track their data related to heart failure, including vital signs, medication
compliance, weight, physical activity and more. By collecting this information remotely at
the patients’ home, the clinicians could identify issues early and adjust their treatment
regimen to prevent the patient from returning to the inpatient setting.
2. Determine the type of innovation. Determine and justify whether the
innovation is sustaining or disruptive.
This program can be classified primarily as a disruptive innovation, as the way these
patients were managed throughout the entire program was changed (InnosightInstitute,
2012). In the previous method of managing heart failure patients, data and patient
compliance to the program were only tracked via in-person visits to the clinic or with
inpatient visits that occurred as a result of exacerbated heart failure symptoms requiring
readmission. In the new process, patient data were recorded and could be viewed by
clinicians in real-time, looking for trends to make incremental adjustments to their
treatment plan, thus avoiding the costly inpatient admission. As the integration of this
technology continues to be a part of the heart failure program, it will likely transition to a
sustaining innovation over time.
