To whom it may concern,
I am writing to you today to report a complaint about one of your patients' safety whom I am a
parent of. Lizzy D was admitted to Wellings NHS, Ward 3B, for a routine surgery on her left
hand. Lizzy notified me that her surgery went smoothly and there were no complications, but I
have my own complaint I would like to voice regarding the inefficient duty of care preformed.
However, it has come to my attention that there is a lack of safeguarding at the hospital. My son
in law recently visited Lizzy and was shocked with the lack of safety at the Hospital as there is a
free entrance into the ward as the door is broken, he has been on two occasions and both times
he has been freely allowed to walk in without any staff being concerned by his presence. This is
immediately alarming as not only is Lizzy a vulnerable person who is on the ward but so are
many other patients, this causes a risk of safety and a possible regard to infection prevention
control. Following the broken door incident, any individual could walk in for example, a
homeless person could walk in who may inhabit an illness, as there are vulnerable patients on
the ward (some just coming out of surgery) this could be potentially life-threatening as they are
exposed to anything and are known to be at a weaker stage. As Well as anyone being able to
gain access onto the ward, anyone could also exit the ward. This is neglect and another
safeguarding issue, Lizzy is exposed to this serious issue and could take it upon herself to leave
as well as many other patients who believe they no longer need to stay in hospital. The issue in
question has caused not only me as a parent to lose trust within the hospital but also Lizzy who
is the individual receiving the professionals care, which is concerning as the service providers
should be able to ensure trust in their community. I believe that as Wellinas NHS trust holds
many exposed patients on this ward that there should be immediate action to remove this risk,
the hospital should fund a new working door that can only be accessed via staff and patients
who are on the ward and receiving care.
Furthermore, I have also noticed that the nurse taking care of Lizzy has a lack of following
policies and procedures, it has come to my attention that Nurse Amina Patel has not been
taking records of medication that has been administered to Lizzy. This is a serious offence as it
is a known law (The Medicines Act) to document any slight uses of medication given to a
patient, this is for many reasons but one being for the patient's safety. For example, as Lizzy
has been given multiple doses of painkillers and if there was another occasion where Lizzy
