Chief Complaint:
60-year-old male presents with a painful rash.
History of Present Illness (HPI):
The patient reports onset of a rash approximately [X days/weeks] ago. The rash is described as
painful, with a burning or tingling sensation before the lesions appeared. The rash is localized to
[specify body region, e.g., right side of chest, back, face] and follows a linear or dermatomal
distribution. He denies similar episodes in the past. He has tried [any over-the-counter creams,
