The first pattern that I noticed in the patient’s mood that helped me
determine the final diagnosis was the manic nature of the “episode” they
described. The shopping, drinking, and grandiose business plan were all erratic,
hyper-focused, and in excess. These seemed classic of manic episodes which made
me consider bipolar I disorder as a top differential. The patient’s description of her
irritable, low mood that has followed and how she seemed to “crash” abruptly was
also another determining factor.

According to DSM-V, criteria for this patient’s final diagnosis must include at
least one manic episode preceded by and possibly followed by hypomanic or major
depressive episodes and not better explained by any other psychotic disorders
(Substance Abuse and Mental Health Services Administration, 2022). It is noted
that major depressive and hypomanic episodes are common in bipolar I disorder
but are not required for its diagnosis (Substance Abuse and Mental Health
Services Administration, 2022).
