HISTORY Chief Complaint: Fatigue and irritability for the past few months. History of Present Illness (HPI): Camilla Franklin, a 48-year-old female, presents with progressive fatigue, mood changes, and irritability over the last several months. She reports difficulty concentrating at work, increased anxiety, and feeling “worn out” despite adequate sleep. She denies recent illness, fever, or infection. Sleep has been restless due to frequent night sweats. She also notes occasional palpitations and unintentional weight loss of about 5 pounds in the last two months. Menstrual history reveals irregular periods over the past year, sometimes skipping months. She denies recent medication changes or illicit drug use. Caffeine intake is moderate, and alcohol use is occasional. Past Medical History: Mild hypertension (diagnosed 3 years ago) No diabetes, thyroid disease, or major surgeries Medications: Lisinopril 10 mg daily Allergies: NKDA (No Known Drug Allergies)
