Supervision - Answer -means the operation of all phases of the business of funeral directing or
embalming under the specific direction of a licensed funeral director or licensed embalmer
Direct supervision - Answer -means the physical presence of a licensed funeral director or
licensed embalmer while the specific functions of the funeral or embalming are being carried out
Board of Embalmers and Funeral Directors - Answer -- 7 members
- appointed by governor
- 5 licensed directors, 4 licenced embalmers, 1 CCO, 2 represent public, 1 at least 60yo
- term is 5 years
When must the preneed annual report be sent in? - Answer -Must be mailed or post marked on or
before due date
Can the board regulate cemeteries? - Answer -No. The board shall license and regulate funeral
homes, embalming facilities, and crematory facilities located at cemeteries
What are the qualifications to get a license? - Answer -- 18yrs of age
- No bad crimes
- Bachelor's degree
- 12mo mort sci program
- 1yr apprenticeship and embalm 25 bodies/direct 25 funerals*
- Completed examination for license
* A FD may substitute 2yr apprenticeship and 50 funerals instead of college
How to reinstate an inactive license? - Answer -May not return to active status for 2 years. Must
complete the following to return:
- File with board
- pass OH laws examination for each license
- $140 reactivation fee
What are the qualifications to get a CCO? - Answer -- 18yrs of age
- No bad crimes
- Completed crematory operation certification program
- CCOs who are actually in charge of the facility do not have to be licensed embalmers/FDs
Where are funeral home/embalming facility/crematory license and embalmer/director licenses and
CCOs displayed? - Answer -In a conspicuous place within the facility. Additionally, the name of
the FD/embalmer/CCO to whom the funeral home license has been issued shall be displayed
immediately on the outside or the inside of the primary entrance used by the public.
Funeral homes should have one of the following: - Answer -Embalming room equipped,
maintained, clean, used only for embalming, preparation, and holding of the dead and containing
articles, facilities, and instruments necessary for those purposes. (Embalming facilities also need
