Chief Complaint/Presenting Problem:
Lack of emotional reactions (numb feeling), loss of hope, poor concentration and
engagement, and suicidal ideation (Symptom Media, 2018).
History of Present Illness:
Nathan reports that he has been experiencing a feeling of numbness and a lack of emotional
reaction for a few months now. He reports that these feelings started about four or five years ago
and often happen when he is stressed. He reports that his younger sister drowned around five or
four years ago in a family pool during summer. Attributed to this event, Nathan's symptoms often
intensify during summer and during the holidays when the family is around. He sometimes blames
himself for the drowning of his sister.
Nathan expresses suicidal thoughts. He expresses that he feels like a burden to everyone
and thinks he can never be fixed. He describes how he believes the world would be better without
him.
The self-blame and guilt he is experiencing are impairing his social life, including his
productivity at work, and having difficulty in focusing generally. This behavior also affects his
