(POs 7 and 8) The practice of nursing is deeply rooted in nursing knowledge, and nursing knowledge is generated and disseminated through reading, using, and creating nursing research. Professional nurses rely on research findings to inform their practice decisions; they use critical thinking to apply research directly to specific patient care situations. The research process allows nurses to ask and answer questions systematically that will ensure that decisions are based on sound science and rigorous inquiry. Nursing research helps nurses in a variety of settings answer questions about patient care, education, and administration. There are several potential questions that could be asked. 2. Identify whether the focus of your question is assessment, etiology, treatment, or prognosis. 3. Remember to integrate references. Professor and class, The case study I choose to discuss is the second one. A 57 y/o female with a history of type 1 DM for 13 years. Treated with biphasic insulin aspart 30/70 after oral meds proved to be ineffective. Patient non- compliant with diet resulting in frequent hypoglycemic episodes and uncontrolled glucose levels. Currently patient is on a new insulin regime with lifestyle modification, which in turn, is leading to more controlled blood glucose levels and fewer hypoglycemic episodes (Kumar, 2015). To formulate a researchable question, we use the PICO(T) format. Our lesson this week explains that when evidence is needed to perform safe effective care you start by formulating a clinical question. Using the PICO(T) format can help to develop this question (CCN, week 1, 2018). P= POPULATION OF INTEREST: sample of subjects you wish to recruit for the study (Riva et al., 2012). In this case our P = Adults with type 1 DM experiencing hypoglycemic episodes while taking short- acting insulin with dietary modifications. I= INTERVENTION OF INTEREST: What treatment will be provided in the study (Riva et al., 2012) In this case our I= Introducing long acting insulin and a new diet and exercise regime. C= COMPARISON OF INTEREST: Also known as the control group. This identifies the group to be used to compare the treatment intervention (Riva et al., 2012). In this case our C= Short-acting insulin with diet and exercise vs. long-acting insulin with diet and exercise. O= OUTCOME OF INTEREST: The result that will be used to measure the effectiveness of the intervention (Riva et al., 2012). In this case the O= Fewer hypoglycemic episodes with well-controlled blood glucose levels. T=TIME: Duration of time that the data will be collected (Riva et al., 2012). In this case the T= 3 months of very closely monitored blood glucose levels. According to our lesson, not every element to this format has to be present, but in this case it will be. MY RESEARCHABLE QUESTION: Will long-acting insulin with regular diet and exercise vs. short acting insulin with regular diet and exercise reduce the amount of hypoglycemic episodes while achieving desire blood glucose levels over the course of 3 months? The answer to this question should provide us with an evidence-based decision about the most effective treatment that will decrease the amount of hypoglycemic episodes along with achieving normal blood glucose levels.
