D396 Task 1
A. For my research question, I will be discussing the outpatient scenario in
which an 85-year-old immigrant is struggling with health issues and a lack of
housing, in addition to having a language barrier between him and
accessible healthcare providers. The question I propose to address the needs
of this client is “For an elderly man with a language barrier and limited health
literacy, does the use of telehealth compared to free clinic visits improve his
health outcomes and understanding of his health status?”
A1. According to Western Governors University (2025), the steps of
evidence-based practice are ask, acquire, assess/appraise, and apply. This
set of steps was used to address the question proposed as follows:
Ask- After using the steps of PICO, I was able to determine my question to
address the needs of the client with an analysis of what an alternative
method of treatment would look like.
Acquire- For my research, I will be collecting external evidence to determine
how effective my proposed method of treatment is. I will research the
benefits of telehealth and how they would be a better fit for my client than
using a free clinic to address his health concerns.
Assess/appraise- I will assess the evidence I have found to determine the
validity and relativity of the research to my own circumstances. If I have
concluded that the source is credible and related to my own research, I will
appraise how confident I am in the results of the research studied and decide
if it will help to answer my proposed question.
Apply- Finaly, I will come up with my plan of how I will carry out my method
of treatment and figure out how I will measure its success. By using DECIDE,
I will be able to execute and evaluate my plan to determine if the use of
telehealth is beneficial to my client as opposed to the use of a free clinic.
, A2. The client’s location, background, and circumstances will affect
outcomes both currently and in the future in many ways. For the client’s
location, he is currently homeless, which can make finding adequate
transportation for appointments difficult as well as finding a safe place to
reside. Telehealth will make it so he does not have to travel often or at all for
doctor’s appointments both currently and in the future. His background of
having low health literacy and a language barrier will make finding in-person
care more difficult, as he would have to find an interpreter or a provider to
convey information to him in his spoken language. Telehealth will make this
far more manageable both currently and in the future, as he will not have to
travel to find someone who can meet his language needs. As he has more
doctor’s appointments that are translated for him, he will gain health literacy
and have a better understanding of how to determine and convey his needs.
His circumstances, such as frequent falls, will hopefully improve over time,
as he may be able to receive medication to prevent his condition from
worsening and possibly improve his condition, as well as acquire mobility
aids that will help prevent him from falling and risking an increase in injuries
that could also worsen his condition in the long-term.
B. According to Lasisi and Adarnijio (2023), it is stated that the use of
telehealth can “significantly improve medical and digital literacy and help in
liberating the understanding of elderly patients to recognize their diagnosis,
accept and adhere to treatment protocols, monitor their progress, and
contribute to their recovery to health.” This emphasizes that the use of
telehealth will help my client to gain a better understanding of his
condition(s) and will help him to improve and maintain his health.

B2. The research information used to form EBP recommendations to address
my client’s needs is good quality, however, none of it references the
treatment of a client who is homeless. Most of the sources discuss the
benefits of telehealth use for elderly clients, especially for the purposes of
accessibility, which my client would greatly benefit from due to his lack of
mobility. By having access to care from a mobile device, it allows him to get
into contact with a health professional wherever he may be, including in the
potential circumstance of a fall.
