Christopher is a teenager who just moved away from his family and may be stressed due to starting college and having to manage his disease. This overall stress and business can be a cause of his unhealthy nutritional status. He may not feel the need to eat or have the will to cook for himself, especially if his mom was the main food preparer in the house. He may be unhappy that he is in the hospital alone with no support from his mom since she has other children she is tending to at home. He may be unreceptive to any information we provide and may need additional teaching. 5. Referring to your feedback log, document the nursing care, including patient education about nutritional needs and therapy, which you provided to Christopher Parrish. Include his response. I reviewed the patient's Electronic medical record prior to entering the room. Once in the room I washed my hands, introduced myself, identified the patient and asked about any allergies, the patient 's response was “none”. I listened to lung and heart sounds, obtained vitals and inquired if the patient was in any pain, patient response was “NO”. I assisted the patient into sitting position prior to administering 5 tablets of pancrelipase as per MAR and HCP orders. I donned gloves and checked the placement of the NG tube prior to aspirating and flushing. Administered tube feeding per MAR and HCP orders. Educated patient on tube feeding, intake and output taking, risk of aspiration, BMI due to most recent weight loss, diagnostic tests that were taken during stay. I then asked the patient about bowel movements and he replied he had two a day mostly formed and soft.
