2. HISTORY
Chief Complaint:
“I'm having episodes of my heart racing.”
History of Present Illness (HPI):
The patient is a 20-year-old female presenting with intermittent
palpitations for several days/weeks. Episodes begin abruptly and last
seconds to minutes. She describes a rapid, pounding heartbeat
sometimes accompanied by lightheadedness, mild shortness of
breath, and anxiety.
There is no chest pain, no fainting, and no triggers such as caffeine,
energy drinks, drugs, or medications. Episodes occur both at rest and
with light activities. No recent illness.
Past Medical History:
• No chronic medical conditions
• No history of thyroid or cardiac disease
