Reason for consultation:
Jimmy Smith whom is currently 9 years of age was referred to me by the remedial teaching
team at Destinata remedial school for a possible diagnosis of Attention-Deficit/Hyperactivity
Disorder (ADHD).
Purpose of consultation:
The purpose of the consultation is to discuss my assessment of Jimmy with his parents, Mr. and
Mrs. Smith, where I will use his time to confer why an assessment by an occupational therapist
is paramount, aid them with additional information on Jimmy’s symptoms, and address any
concerns that they might have. This consultation report with Jimmy’s parents will be shared
with the remedial teaching team to assist them with planning an appropriate intervention
strategy for Jimmy.
Consultation format:
The consultation entailed a 60-minute appointment in the boardroom of the Destinata remedial
school with Mr. and Mrs. Smith.
1. Description of the client
Mr. and Mrs. Smith is a married couple in their early 40’s and the biological parents of Jimmy
Smith. Mr. Smith is a tall man with brown hair and green eyes, whereas Mrs. Smith is a
concerned mother with long red hair and pure blue eyes.
