Pediatric V-Sim Care Plan EM V-Sim Pediatric Nursing Care Plan Eva Madison Gastroenteritis Stockton University Camryn Kaenzig Background Information and Growth and Development The virtual simulation introduced the patient Eva Madison, a five-year-old Caucasian female diagnosed with dehydration upon admission to the emergency department. Eva’s mother, Heather Madison, brought her to the emergency department after a three-day long episode of vomiting, diarrhea, and not being able to keep fluids down nor no urination since the day before admission. According to the electronic health record on Eva’s simulation, Eva has lost 0.7 kilograms since a couple months ago at her last well-visit checkup. Eva is currently 120 centimeters tall while her head circumference was not recorded un her records. An intravenous bolus of saline of 400 mL was started in the emergency department and has just finished prior to performing an assessment of the patient. To add, there was not much information gathered within the virtual simulation and the electronic health records that suggest a thorough family structure. Specifically, EM’s family consists of her mother, HM. It is unknown what other family members are present in EM’s life. However, HM is EM’s legal guardian and care provider. The CVC Growth Charts applicable for a child over the age of two years old to about twenty years old was used to plot the expected height and weight for a child of five years old. The expected findings for a five-year old is 100-116 cm (from 5th to 95th percentile) for height and 14-24 kg (5th to 95th percentile) for weight (Growth charts - WHO child growth standards., 2019). Below I have plotted the expected height and weight for a child of five years of age. Plot: For a five-year-old, in specific, there are expected vital sign findings. Heartrate in between the beats per minute of 65-: 80-120, a blood pressure between the intervals of 89-112 systolic and 46-72 for diastolic, a respiratory rate of about 20-25 breaths per minute, and temperatures of (tympanic: 35.8-38 degrees Celsius,) (oral: 35.5-37.5,) (axillary: 36.5-37.5), and an oxygen saturation of SpO2: 93% (same as adults). (Pediatric Vital Signs Reference Chart, n.d.). - - - - - - - - - - -- - - -- - - - - - - - - - - - Possible equipment needed at home post-discharge is a measurable urine cup for output values (Family and patient should be educated on normal values). Possible medications needed at home for this client are Zofran or other antiemetics (for N/V,) a multivitamin (including potassium, calcium) and other fluids that should match her total fluid intake daily.
