,Voorwoord

Mijn naam is Tessa Eilders en ik werk binnen het team ontwikkelingsstoornissen van Dimence. Binnen
dit team staan ADHD en autisme centraal, maar in de praktijk zie je vaak dat cliënten te maken
hebben met een combinatie van verschillende psychische problematieken.

In mijn rol combineer ik verpleegkundige behandeling met de functie van zorgcoördinator. Ik ben
betrokken bij het opstellen en uitvoeren van behandelplannen, stem de zorg af met verschillende
disciplines en begeleid cliënten in hun dagelijks functioneren.
