History of Present Illness (HPI):

for the patient.
Consider asking about the patient’s perspective: “Tell me how that makes you feel.” “How
does this affect your life?” “What symptom is the most distressing for your?”

-Grades have been slipping in school. She’s a little forgetful at home too.
-She has trouble concentrating at school. Her teachers mentioned she interrupts class and
talks excessively.
-Does not like to do homework and has problems concentrating on homework. States
“homework and school are so boring.”
-Always had problems concentrating in school. Increased over the past 7 months and
grades are slipping
-No sudden change in behavior or personality
-No history of mental health problems
-Denies difficulty sleeping and fatigue. Sleeps 8 to 9 hours per night.
- Eating a balanced diet and eats most meals at home. No recent changes in appetite.
-Doing well socially and making friends
-Caffeine and chocolate intake are limited by parents
-Has desire to do well in school.
-Likes to talk to friends a lot during class. It distracts her from concentrating at school.
-Teachers say she interrupts them and friends too much sometimes
-States “I get all wrapped up in talking with my friends in class, so I guess that makes it hard
to listen to my teachers.”
-Denies feeling anxious. Feels happy all the time. Does not feel like she is sad.
