Chief Complaint: Back Pain
History of Present Illness
I. When did your back pain begin? “It started yesterday. I took two tylenol, and they
didn’t help (Reteguiz, 2004)”.
II. What were you doing when your back pain began? “It started when I moved my
computer monitor to my new office (Reteguiz, 2004)”.
III. What is the quality of the back pain? “It is dull and throbbing (Reteguiz, 2004)”.
IV. Does the back pain radiate anywhere else in your body? “No. It’s staying right here, in
the middle of my lower back (Reteguiz, 2004)”. (The patient points to their L4-5 area).
V. What alleviates the back pain? “Lying still on my back or lying on one side with
my knees bent helps a little (Reteguiz, 2004)”.
Past Medical History
VI. Have you had any back problems before? “No.”
VII. Besides the two tylenol that you took yesterday, have you been taking any other over-
the- counter, prescription, or herbal medications? “No.”
Family History
VIII. Is there a history of ankylosing spondylitis in your family? “No.”
IX. Is there a history of other spine or back problems in your family? “No.”
