History of Present Illness

I. How did you injure your wrist? “I felt down the stairs. It was an accident (Reteguiz,

2004).”

II. When did this injury happen? “About one hour ago (Reteguiz, 2004).”

III. On a 0-10 scale with 10 being the worst pain imaginable, how is your pain right now?

“7.”

IV. What is the quality of the pain? “It is throbbing.”

V. Do you have any other injuries at this time? “I have a few bruises, but nothing major

(Reteguiz, 2004).”


Past Medical History

VI. Do you have any other medical issues? “No.”

VII. Are you currently taking any over-the-counter, prescription, or herbal medications? “No.”


Family History

VIII. Is there a history of osteoporosis in your family? “No.”

IX. Is there a history of cancer in your family? “No.”


Social History

X. Do you feel safe at home? “Not really. I never know when my husband is going to

become angry with me. He gets angry when he drinks, and he drinks almost every day

(Reteguiz, 2004).”
