Chief Complaint: Epigastric Pain
History of Present Illness:
I. When did the abdominal pain begin? “My abdomen has been in constant pain for 2 days.”
II. Does the abdominal pain radiate anywhere? “It radiates to my back.”
III. What are the characteristics of the abdominal pain? “It’s sharp and stabbing, like a
knife was run through me.”
IV. Are there any factors that aggravate the pain? “Eating or drinking makes the pain worse.
Lying flat makes the pain worse.”
V. Are there any factors that relieve the pain? “The pain goes down when I lay on my side.”
Past Medical History
VI. Have you felt abdominal pain before? “Yes, when I had pancreatitis two years ago
from drinking.”
VII. Are you currently taking any medications? “No.”
Family History:
VIII. Is there a history of cancer in your family? “No.”
IX. Is there a history of pancreatic disease in your family? “No.”
Social History:
X. Do you drink alcohol? “Yes. I drink ten cans of beer every day.”
XI. When was your last drink? “Yesterday, around 16 hours ago.”
