Chief Complaint: Left-hand Tingling
History of Present Illness:
I. When did your left hand begin tingling? “It started last night before I went to bed, and
now it is worse (Reteguiz, 2004)”.
II. Is the tingling concentrated in one area of your hand? “Yes. It is worse in my middle
and index fingers (Reteguiz, 2004)”.
III. Does the tingling radiate elsewhere? “No.”
IV. What makes the tingling worse? “Sleeping and driving (Reteguiz, 2004)”.
V. What alleviates the tingling? “Shaking my hand helps a little (Reteguiz, 2004)”.
Past Medical History:
VI. Do you have rheumatoid arthritis? “No.”
VII. Do you have diabetes? “No.”
Family History:
VIII. Is there a history of cerebrovascular accident in your family? “No.”
IX. Is there a history of diabetes in your family? “No.”
Social History:
X. Do you have to perform repetitive movements in your daily life? “Yes. I work full-
time as a secretary, and I’ve done data entry on a keyboard all day, every weekday, for
years (Reteguiz, 2004).”
