Week 1

This was the first week that my mentor and I arranged to meet and plan on how I could

access the needed clinical and nursing resources to successfully undertake my capstone project.

To gain some useful clinical experiences, my mentor took me around the hospital facility and

introduced me to important staff members who would later assist in information gathering. While

here, I learned that some nurse educators are involved in data and information collection from

rural communities on diabetes mellitus patients. Most importantly, I visited the Telehealth and

Telemedicine department as it could play a crucial role during this practicum period. One thing

my mentor emphasized during this period was the need to use only evidence-based articles for

purposes of research, information, and data collection. I found out that in clinical and nursing

practice, adopting an evidence-based approach is the best in problem-solving (Melnyk et al.,

2005). Evidence-based practices has equally become the new standard in nursing practice as it

facilitates quality and timely decision-making processes. At the end of this week one, I had

managed to successfully build a very strong mentor-mentee relationship for that could be crucial

in realizing the goals set out in the practicum project.

Week 2

It was the week that I focused on the major aspects of my change proposal as I identified

key areas of research. I was assisted by my mentor to find out the major evidence-based articles

and studies related to rural communities affected by diabetes mellitus. This was also

characterized by finding out how telemedicine programs have assisted the rural population in

remote counties receive clinical and nursing care as they cannot easily access the recommended

affiliate and medical centers. I used the GCU Library as my primary point of reference to find

out the relevant scholarly articles. My mentor intimated to me that there are three major levels of
