MEMORANDUM

Subject: Fatality Motor Vehicle Collision
Incident Overview
On August 8th, at approximately 4:45, a University City Fire Rescue employee (Michael
O. Leary) was involved in a motor vehicle collision (MVC), resulting in the death of a Universal
County resident (Julie Doe). (CSU, 2021) Leary was operating a fire department vehicle while
returning to our district from training when the MVC occurred. Leary was eastbound on
University City Pike, just inside the city limits of University City, and was passing a line of
traffic at a speed above the posted limit when a car attempted to make a left turn into the
driveway of a residence and was struck broadside. The vehicles traveled 158 feet after the
impact, with the front of the F250 pickup driven by Leary embedded in the driver's side of the
Dodge Intrepid driven by Doe. A number of witnesses have stepped forward, claiming that, at the
time of impact, Leary was traveling anywhere between 50 and 70 miles per hour.
