Presenting Problems:
The client was referred by his father for the management of the following presenting complaints:
• Can count a pile of fallen toothpicks without counting all of them individually
• I need everything in order and have to follow a routine
• I cannot fly because all airlines have crashed some time except one
• I see it, I can solve complicated math equations

• I don’t like too much noise
• I don’t like it when people touch me. When my brother hugs me
• I have an attach when something scary happens
• I cannot make decisions
History of Presenting Problem:
The client displayed symptoms when he was less than 5 years old.
Personal History:
The client did not mention birth complications or prenatal history. In unusual events the
client reported the death of his mother when he was still a child and also his being sent to the
institute which meant being separated from his family. Both incidents happened in the same month.
He was scared of hot water because he thought he would burn his younger brother. He was
scared of flying in an airplane because he remembered all the dates when planes had crashed.
Medical History:
Client reported that he was sent to an institute for psychological help by his father into on
January 21st, 1965. No other medical history was obtained.
Prior Mental Health Treatment:
Did not identify what the treatment exactly was other than being sent to the institute on
January 21st, 1965. “It was Thursday. Very snowy out. 7.2 inches. Snow that day.” He said there
was a “short and sudden illness” which is why he was sent. According to him he was sent because
they were afraid he would hurt his younger brother.
Psychiatric Medication:
Was not taking any psychiatric medication.
