1. A nurse is caring for a client newly diagnosed with pertussis. What is one(1) intervention
the nurse will incorporate in the plan of care.
▪︎Educate about coughing, breathing and promote effective coughing.
•Teach the patient the proper ways of coughing and breathing. (e.g., take a deep breath,
hold for 2 seconds, and cough two or three times in succession).
•Educate the patient about optimal positioning (sitting position), use of pillow or hand splints
when coughing, use of abdominal muscles for more forceful cough, use of quad and huff
techniques, use of incentive spirometry, and importance of ambulation and frequent position
changes.
2. A client threaten to leave the hospital against medical advice and the nurse notes that
this is the client's right. In addition to having the client sign an Against Medical Advice form,
what is the nurse's priority action?
▪︎To protect you and other health care providers caring for the patient and your facility if
problems were to arise from an unapproved discharge, you should have the patient sign an
AMA form. This form should clearly document that the patient knows he's leaving AMA, that
he's been advised of and understands the risks of leaving, and that he knows he can come
back. Use his own words to describe his refusal to stay for further treatment.
▪︎Here's what to include on the AMA form:
•explanation of the risks and consequences of the AMA discharge, as told to the patient,
and the name of the person who provided the explanation.
•other places the patient can go for follow-up care.
•names of people accompanying the patient at discharge and the instructions given to them.
•patient's destination after discharge.
▪︎Relate the patient's state
•In the progress notes, document statements and actions that reflect the patient's mental
state at the time he left your facility.
~This helps protect you and other health care providers caring for the patient and the facility
against a charge of negligence if the patient later claims that he was mentally incompetent
at the time of discharge and was improperly supervised while in that state.
▪︎The AMA form should document that the patient understands the risk of leaving against
medical advice and knows he can come back.
3. A client newly admitted with abdominal pain ask the nurse to explain how their blood test
indicating that they are positive for helicobacter pylori has to do with their abdominal pain.

▪︎Helicobacter pylori (H. pylori) infection occurs when H. pylori bacteria infect your
stomach.
▪︎It can damage the tissue in your stomach and the first part of your small intestine (the
duodenum).
▪︎This can cause redness and soreness (inflammation). In some cases it can also cause
painful sores called peptic ulcers in your upper digestive tract.
▪︎Ulcer symptoms may include belly or abdominal pain.
4. A nurse manager is preparing for a performance appraisal review for an employee who
has been identified as substandard. Describe the process of conducting a performance
appraisal review and when disciplinary action is necessary.
▪︎You should be given the chance to sit down with your manager or supervisor at least once
a year and have a performance appraisal. This is an opportunity for you to look together at:
•how you've developed over the last year, particularly around any specific objectives you'd
agreed previously.
•whether you're still producing the necessary level of performance in relation to the
competencies set out for your role.
•what education and training opportunities you're going to need over the coming year.
•producing a personal development plan for you (which we'll look at shortly).
•setting specific objectives for you to achieve over the coming year.
▪︎The objectives you agree with your manager should be what is called SMART, that is:
•Specific (you should understand exactly what the objective is).
•Measurable (you and your manager should be able to look back in a year's time and
actually be able to see whether the objectives have been achieved or not)
•Agreed and achievable (you should be happy with the objectives and they should not be so
challenging that you could never achieve them).
•Realistic and relevant (the objectives should reflect your skills, experience and grade and
should relate directly to the work you do).
•Time-related (you and your manager should set realistic time-scales over which the
objectives will be achieved).
