Procedural Checklist in Assessing the Breast and Lymphatic System

General Objectives: Given simulated situations, the students will be able to systematically assess, observe and
perform assessment of the breast and lymph nodes.
Specific Objectives:
1. Identify and perform the steps of assessing the breast and lymphatic system.
2. Identify the techniques of physical assessment to be applied in assessing the breast and lymph nodes.
3. Determine the normal findings from the deviations in performing the procedure.
4. Perform accurate documentation.

.50 Satisfactory Performed the procedure but requires close supervision and shows
potential for improvement.
.25 Needs Failed to perform the procedure, unable to function well and needs
Improvement repeated specific/ detailed guidance or direction.
CRITERIA:

1. Prepare all the materials/supplies to be used.
-----Towel, Drape, Gloves, Centimeter Ruler, teaching
aid for BSE (Breast Self-Exam) Introduce self and verify the client's identity.
Explain procedure to client. Perform hand hygiene and observe appropriate
infection control procedure. Provide for privacy, expose only the body part
to be examined.
2. Inquire if the client has a history of breast masses, pain or tenderness in the
breast, medication history and risk factors that may be associated with
development of breast cancer.
3. Inspect the breast for size, symmetry, and contour or shape while the client
is in sitting position.
