physiological disorders.


There are different individuals who can plan a care pathway for a patient that is
suffering from different types of conditions. For example, someone that has a
hypertension would have a different care pathway from someone that has been
diagnosed with Alzheimer’s dementia.

If you think that you might have a high blood pressure because of your poor
well-being and you might experience the general symptoms of high blood
pressure then it is recommended to make an appointment with your GP or family
doctor. They usually have the wide range of knowledge on how to deal with
hypertension and how to minimise the effects of it on your health. There are
several options which your GP might suggest for you to undertake. This includes
making changes to your lifestyle. So for example, try to cut down on fast foods,
possibly minimise the dairy intake and unhealthy junk foods. The doctor might
identify that the poor diet might be the result of high blood pressure, which is
why they might direct you to visit a dietician who is more experienced on this
subject.

The dietician would look over your whole intake food plan and would construct a
healthier food plan which you would have to try and follow. The dietician would
take into the account your food allergies if you have any, and also any specific
preferences.

The doctor also might realise that the cause of hypertension in a patient is stress
which could make the person crave and eat the unhealthy foods. This results in
the gain of weight and quite possibly high blood pressure. In this case, the doctor
might direct the patient to visit a therapist who would carry out weekly sessions
where he and the patient would discuss any problems the person has.

The therapist would talk to the patient about his problems which cause the stress
to occur. They then together would manage the issues by discussing ways in
which they could deal with them. For example, if the person is stressing about
not having enough time to see their friends etc. or they might even be depressed
then the therapist would talk to them about how they could organise their time
in order to be able to socialise.

All of these professionals work together to provide the best care for the patient.
The things that they would carry out to work in a team would be communication.
For example, doctor might report to the dietician or the therapist any changes for
example; the person might begin to have a lower blood pressure which means
