OB Content Exam 2 Antepartum Complications
Bleeding during pregnancy -first trimester ---spontaneous abortion (loss) ---ectopic pregnancy Second trimester -gestational trophoblastic disease -Third Trimester (placenta problems) ---placenta previa ---placenta abruptio -vasa previa Other causes of bleeding -recurrent premature dilation of the cervix -painless bleeding -cervical dilation -premature expulsion of the fetus preterm labor: -bloody vaginal discharge -uterine contractions tha cause cervical dilation and effacement Hydati from male -benign proliferative growth of the placenta trophoblast spontaneous abortionnaturally occurring termination of pregnancy; also known as a miscarriage -pregnancy terminated before 20 weeks gestation *requires a death certificate management of abortion -Assess pain, bleeding, LMP, VS, medical hx labs:HCG, CBC, ultrasound, and blood type Rh bedrest-supportive care discharge teaching BP=decreased if bleeding HR=increase if bleeding Pulse ox=decreased if bleeding Threatened abortion vaginal bleeding without dilation of the cervix or passage of tissue -placenta is still attached to uterine wall -doesn't mean its over-but requires a lot of follow-up patient presentation: -cramping, backache -vaginal bleeding -HCG level and VS due to check viabilityInevitable abortion placental separation from uterine walls - a lot of bleeding- end w/loss of pregnancy because of placenta separation Patient presenation: -cramping, increased bleeding -cervix dilated -membrane may rupture (bag of waters) incomplete abortion Embryo or fetus(products of contraception Poc) are passed but the placenta remains in the uterus -no longer embryo fetus present Patient Presentation -bleeding, cramping -cervix dilated -hemorrhage is possibleBleeding in pregnancy other symptoms backache abdominal pain rupture of membranes dilation of cervix fever signs and symptoms of hemorrhage hypotension tachycardia passage of clots/or tissue-send for examination lab tests; hgt, hot, clotting factors, wbc serum human chorionic gonadotropin (BHCG)-confrm pregnancy Diagnostic/procedures -ultrasound: determine viable or non viable
