abilities to better support their growth and learning in our Grade 1 class. Please take
a few minutes to complete this questionnaire.
Question 1: Does your child currently participate in any physical activities or sports
outside of school (e.g. swimming, dancing, soccer, etc.)?
Reason: We want to understand if your child has any prior experience with physical
activities that may impact their ability to participate in our physical education
program.
Question 2: How would you describe your child's overall energy level (e.g. high
energy, low energy, average)?
Reason: This information will help us plan physical education activities that are
tailored to your child's energy level, ensuring they are able to participate safely and
successfully.
Question 3: Has your child ever had any injuries or health concerns that may impact
