focused on providing the best and most effective care to our patients. It is the coordination of
patient care activities and information communication by all the stakeholders involved in a
patient's care to promote safety and quality of care. This is important in managing our clients'
health, especially those with complications or more than one disease. The goal for today is to
define what care coordination is, what strategies can be used in the process, and why nurses
are such an essential part of it. Let's embark on this journey to understand better and
implement care coordination in our practice.
Effective Strategies for Collaborating with Patients and Families
Collaboration in healthcare is defined as a partnership between patients, families, and
healthcare professionals aimed at achieving the best possible health outcomes. Several
strategies can effectively be employed to collaborate with patients and families. One such
strategy, which is important in collaborating with patients and their families, includes drug-
specific educational interventions (Ribeiro & Schlindwein, 2021). Proposed interventions,
such as individualized regimes of medication consumption and telephonic checkups, could
play a significant role in furthering the program in terms of compliance with medication
