A. Assess your ability to gather information on your client within the iHuman Virtual
Patient Encounter.
1. Use the iHuman score sheet to review your results for the focused health history
and focused physical exam. How did you perform?
Based on the iHuman score sheet, I performed well overall on both the focused
health history and focused physical exam. For the history portion, I achieved
100%, asking 35 questions with 9 correct and no missed questions, which
indicates a thorough and accurate approach. For the physical exam, I scored
94%, performing 23 exams with 14 correct, 1 partially correct, and no missed or
harmful actions. This shows that I was able to conduct a comprehensive and safe
physical assessment, though there is a small area for improvement in accuracy.

history and focused physical exam sections of the case?
I found the focused health history section relatively easy to navigate, especially
since the system clearly guided the flow of questioning. However, the physical
exam section was a bit more challenging, as it required remembering the most
relevant assessments for the case and performing them in a logical order
without missing key steps.
3. Describe one strategy to improve your performance in the next Virtual Patient
Encounter.
One strategy I plan to use to improve my performance in the next Virtual Patient
Encounter is to review the most common physical exam techniques and their
indications prior to starting the case. This will help me focus on performing the
correct exams more efficiently and accurately, reducing the chance of partial or
incorrect assessments.
B. Assess your performance in documenting your findings on the electronic health record
(EHR).
1. What did you find easy or difficult about navigating through the documentation
of the history and physical within the system?
I found it fairly easy to input the basic information into the EHR, as the system was
organized into clear sections for history and physical findings. However, it was a bit
challenging to ensure that all relevant details were entered accurately and concisely,
especially while balancing clinical reasoning and time management during the case.
2. Did you document all required components in the case?
Yes, I documented all the required components of the case, including the patient’s
history of present illness, past medical history, review of systems, and focused
physical exam findings. The system showed completion of necessary documentation
fields, though I recognize there may still be areas for refinement in clarity or clinical
depth.
